Requirements

  • An hbo-diploma in occupational therapy (ergotherapie) or social work (maatschappelijk werk)
  • Experience with the rehabilitation target group and/or working in a rehabilitation institution is a plus (pré)
  • Knowledge of or affinity with social security
  • Knowledge of or experience with guiding reintegration trajectories
  • Good networker who seeks collaboration with others where possible
  • Ability to work independently (solistisch) with a healthy dose of entrepreneurial spirit
  • Good written and verbal communication skills
  • Ability to assess the client's limitations and possibilities and translate them into labor market opportunities

Responsibilities

  • Organize information sessions and/or labor consultations at the Leiden location and, in consultation, at other Basalt locations within adult rehabilitation
  • Conduct intake interviews with clients for the start of private services
  • Guide the entire labor reintegration trajectory, supporting the client in taking appropriate steps toward work resumption
  • Maintain contacts with other involved parties, such as the employer and occupational physician, and conduct workplace visits
  • Advise on next steps if reintegration deviates from expectations, e.g., referral to an occupational physician or rehabilitation physician
  • Prepare (independent) interim and final evaluations reflecting the client's physical and mental capacity
  • Determine next steps for the client after completing the trajectory
