Requirements

  • RN CAP 2 designation (entry level for new employees meeting criteria or advancement from CAP 1)
  • Accountability for provision and coordination of clinically competent care (assessment, planning, intervention, evaluation) for assigned patients
  • Ability to delegate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership, and quality improvement activities
  • Completion of CAP requirements to maintain or advance
  • Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation (with equipment aids/coworkers)
  • Frequent, prolonged physical activity in direct patient care environment

Responsibilities

  • Perform all essential functions under direction of Supervisor and guidance from Educator, Preceptor, or in collaboration with another RN
  • Perform ongoing, systematic collection and analysis of patient data pre-during-post treatment, document in medical record, adjust treatment plan, notify Supervisor/Physician/primary nurse as indicated
  • Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education
  • Recognize and report/collaborate on abnormal patient status with health team members (Supervisor, physician, contracted facility/primary Nurse)
  • Direct and provide safe, effective patient care for assigned patients per modality-specific treatment procedures
  • Assess daily patient care needs and collaborate with direct/ancillary staff
  • Initiate and coordinate communication with FKC/Non-FKC dialysis providers and contracted facility personnel for continuity of care
  • Implement, administer, monitor, and document intradialytic transfusions; notify physician/blood supplier of adverse reactions
  • Administer medications as prescribed, document justification and effectiveness
  • Initiate and assist with emergency response measures
  • Serve as resource, leader, coach, mentor, role model for employees; set examples of behavior/work habits/attitudes
  • Provide specialized nursing care instructions to hospital/facility staff as per contract
  • Assist in Outpatient facility as needed
  • Serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
