Entry level designation into Clinical Advancement Program (CAP) for new employees meeting RN CAP 2 criteria or advancement from CAP 1
Accountability for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Ability to delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership functions, and quality improvement activities
Completion of CAP requirements to maintain or advance
Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing

Responsibilities

Perform all essential functions under direction of Supervisor and guidance from Educator, Preceptor, or in collaboration with another RN
Perform ongoing, systematic collection and analysis of patient data pre-during-post treatment, document in medical record, adjust treatment plan, and notify Supervisor, Physician, primary nurse as indicated
Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education
Recognize and report patient status variations from normal to appropriate health team members
Direct and provide safe effective patient care for assigned patients as prescribed for modality-specific treatments
Assess daily patient care needs and collaborate with direct and ancillary staff
Initiate and coordinate communication with FKC and Non-FKC dialysis providers and contracted facility personnel for continuity of care
Implement, administer, monitor, and document patient's response to intradialytic transfusions; notify of adverse reactions
Administer medications as prescribed and document justification and effectiveness
Initiate and assist with emergency response measures
Serve as resource, leader, coach, mentor, and role model for employees
Provide specialized nursing care instructions to hospital/facility staff as per contract
Assist in Outpatient facility on as-needed basis
Serve as Preceptor to new employees
Perform all other duties as assigned by Supervisor
