Responsibilities

Lead collaboration among interdisciplinary team to develop, coordinate, and facilitate direct social services interventions for assigned patients and support systems
Conduct individual, group, and family therapy sessions using empirically supported techniques (e.g., CBT, DBT, family therapy)
Conduct comprehensive assessments to identify patients’ mental health concerns, strengths, and challenges
Develop individualized treatment plans based on patients’ needs and goals; facilitate treatment team meetings and guide implementation of clinical interventions
Provide counseling and support incorporating evidence-based techniques to manage symptoms, improve well-being, and address in-the-moment needs in the milieu
Educate patients on mental health/substance use conditions, self-care strategies, treatment options, costs/benefits of engagement, and aftercare plans
Document patient progress, therapy sessions, goals, and interventions within required timeframes
Collaborate with treatment team members (e.g., psychiatrists, social workers, case managers) to coordinate care and ensure optimal outcomes
Adhere to ethical guidelines, maintain confidentiality, and create a safe/supportive environment
Participate in ongoing professional development and continuing education
Perform other duties as directed by manager/clinical operations supervisor to support patient care and system initiatives
