Requirements
- Adheres to FMCNA Compliance Program, including all regulatory and FMS policy requirements
- Functions under direct supervision of a licensed social worker
- Complies with FMCNA policies, procedures, training, and regulations set by corporation, state, and federal agencies
- Assists in maintaining a safe and clean working environment
- Supports FMCNA Quality Enhancement Program (QEP) and CQI activities, including patient satisfaction and process improvement
- Meets minimum required field education hours established by CSWE
- Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation
- Accountable for outstanding customer service to all external and internal customers
Responsibilities
- Drives FMS culture through values and customer service standards
- Develops and maintains effective relationships through effective and timely communication
- Takes initiative to respond, resolve, and follow up on customer service issues
- Assesses patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
- Participates in care planning with patient and interdisciplinary team to identify interventions for rehabilitation, treatment goals, and quality of life
- Utilizes FMS Ultra Care patient education programs, social work theory, methods, and quality of life instruments for assessment and care planning
- Provides outcomes-driven psycho-educational counseling to improve treatment adherence, vocational/educational rehabilitation, and quality of life
- Provides supportive and goal-directed counseling for patients seeking transplant
- Assesses patient awareness of advance directives, assists with forms/information, and facilitates discussions with team and family
- Provides general information about Do Not Resuscitate Orders and Advanced Directives
- Refers patient and team to community resources (e.g., home health, vocational rehabilitation) for optimal outcomes
- Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
- Assesses patient knowledge of kidney disease for barriers to treatment adherence
- Provides tailored education on kidney disease and treatments with patient, family, and healthcare team
