Requirements

Part-time student intern in Social Work pursuing field education hours as established by CSWE
Functions under direct supervision of a licensed social worker
Adheres to FMCNA Compliance Program, policies, procedures, training, and regulatory requirements (corporation, state, federal)
Knowledgeable of FMCNA Social Work Policy, including documentation
Knowledgeable of FMS Ultra Care patient education programs, social work theory, methods, and quality of life measurement instruments

Responsibilities

Supports FMCNA's mission, vision, core values, customer service philosophy, Quality Enhancement Program (QEP), CQI activities, and process improvement for patient satisfaction and QEP goals
Assists in maintaining a safe and clean working environment
Drives FMS culture through values and customer service standards; provides outstanding customer service to external and internal customers
Develops and maintains effective relationships through timely communication; takes initiative to respond, resolve, and follow up on customer service issues
As part of interdisciplinary team, assesses patients' psychosocial status, strengths, and needs affecting rehabilitation and treatment outcomes
Participates in care planning with patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
Provides outcomes-driven psycho-educational counseling to improve treatment adherence, vocational/educational rehabilitation, and quality of life
Offers supportive, goal-directed counseling to transplant-seeking patients
Assesses patient awareness of advance directives; assists with forms/information and facilitates discussions with team, patient, and family/support persons
Provides general information about Do Not Resuscitate Orders and Advanced Directives
Refers patient and team to community resources (e.g., home health, vocational rehabilitation) for optimal outcomes
Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
Assesses patient knowledge of kidney disease for adherence barriers; provides tailored education with patient, family, and team
With interdisciplinary team, provides information about all treatment options
