Requirements

Master's in Social Work (MSW) - entry level role
Current knowledge of local vocational/educational rehabilitation programs
Knowledgeable of and adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy
Ability to utilize Social Work Theory of Human Behavior and accepted methods of social work practice
Scope of clinical training and state license permitting supportive counseling services

Responsibilities

Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
Participate in care planning with the patient and interdisciplinary team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
Utilize FKC patient education programs, social work theory, methods, interventions, and quality of life instruments in assessment and care planning
Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
Provide supportive counseling services to patients within scope of clinical training and state license
Provide educational and goal-directed counseling to patients seeking transplant
Provide information and assist with referrals to community resources (e.g., home health services, vocational rehabilitation)
Assist patients with referral and access to vocational rehabilitation to support employment or education
Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions
Participate in discussions of patient DNR status with physician, nurse, patient, and family
Document MSW interactions and interventions provided to patient and/or family
Provide psychosocial support and/or Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns
Participate in monthly Quality review meetings and report on quality indicators (e.g., adherence, Missed/Shortened Treatments, Quality of Life Trends, Service Recovery)
Assess patient knowledge of kidney disease for barriers affecting adherence and provide tailored education with patient, family, and team
