Responsibilities

  • Create a single point of contact for transferring patients to the most appropriate Avera hospital, collaborating on unique patient needs and facility identification
  • Support assignment, tracking, and problem-solving for timely patient placement
  • Work collaboratively with team members, nursing departments, outlying facilities, and physicians to facilitate efficient patient movement
  • Gather clinical data to ensure continuity of patient care and appropriate patient placement
  • Monitor all transfers, identify potential delays in care, and provide communication between care team members
  • Oversee patient flow from outlying facilities and clinics for efficient and seamless entry
  • Prioritize and place patients in appropriate regional sites using critical thinking skills and department policies/guidelines
  • Act as a liaison with bed placement coordinator to ensure timely patient transfers
  • Evaluate the transfer center process toward attainment of outcomes
  • Collaborate with physicians, nurses, health care providers, and specialists in assessing patient placement
  • Utilize research/evidence-based findings in practice if appropriate
  • Recognize and integrate the use of healthcare technology and/or information management systems
