Registered Nurse (RN) license from the Board of Nursing - active license in state of practice upon hire
Visual acuity adequate to perform position duties
Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds

Responsibilities

Create a single point of contact for transferring patients to the most appropriate Avera hospital, collaborating on unique patient needs and facility identification
Support assignment, tracking, and problem-solving for timely patient placement
Work collaboratively with team members, nursing departments, outlying facilities, and physicians to facilitate efficient patient movement
Gather clinical data to ensure continuity of patient care and appropriate patient placement
Monitor all transfers, identify potential delays in care, and provide communication between care team members
Oversee patient flow from outlying facilities and clinics for efficient and seamless entry
Prioritize and place patients in appropriate regional sites using critical thinking skills and department policies/guidelines
Act as a liaison with bed placement coordinator to ensure timely patient transfers
Evaluate the transfer center process toward attainment of outcomes
Collaborate with physicians, nurses, health care providers, and specialists in assessing patient placement
Utilize research/evidence-based findings in practice if appropriate
Recognize and integrate the use of healthcare technology and/or information management systems
