Requirements

  • Licensed Practical Nurse (LPN) - active license from the Board of Nursing in the state of practice upon hire (RN equivalent also accepted)
  • Ability to work casual/PRN schedule: minimum 2 shifts per month (one weekend shift: Saturday or Sunday), one holiday per year if needed
  • Ability to perform essential job functions satisfactorily, including working specified hours

Responsibilities

  • Provide professional nursing care, assessments, and evaluations to residents within scope of practice
  • Develop nursing care plans by obtaining medical histories, examining residents, documenting physical/psychological/social conditions, conferring with family/friends, establishing diagnoses/objectives/interventions/schedules
  • Review physician prescriptions, communicate nursing care plans and pertinent information to healthcare team, coordinate with other providers
  • Provide physical, emotional, psychological, and spiritual support to residents, families, and friends; teach healthcare requirements including conditions, medications, self-care skills, and resources
  • Evaluate resident care by adhering to standards, measuring outcomes, reviewing records, noting changes, re-evaluating/modifying care plans
  • Maintain safe, secure, healthy environment by following infection-control, medication administration/storage, controlled substance regulations, organizational policies, and legal standards; alert others to concerns
  • Maintain resident care equipment: follow instructions, troubleshoot, perform preventive maintenance, call for repairs
  • Manage resident care supplies: check stock, anticipate needs, order/expedite/verify/stock items
  • Coordinate admissions, discharges, and transfers to/from the facility
  • Direct, plan, and coordinate workflow of nursing assistants and other nursing personnel
  • Maintain competence in nursing capabilities as outlined by leadership
