Requirements

  • Registered Nurse (RN) license
  • Entry-level designation into Clinical Advancement Program (CAP 1)
  • Ability to perform under direction of Supervisor with guidance from Educator, Preceptor, or in collaboration with another RN
  • Complete CAP requirements to advance
  • Physical ability for direct patient care involving heavy lifting, moving equipment and patients, and assisting with ambulation (reasonable accommodations for disabilities)

Responsibilities

  • Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust treatment plans as indicated, and notify Supervisor or physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapy
  • Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource for health care team, participate in staff training and orientation of new staff
  • Ensure correct laboratory collection, processing, shipping, and reschedule missed or insufficient collections
  • Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate’s home environment, and partner/family readiness and ability
  • Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies via organized training program
  • Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art programs and equipment
  • Conduct initial home visits with another staff member present
  • Participate in decision-making, teaching, leadership, and quality improvement activities
  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, nursing standards, state and federal regulations
  • Perform all other duties as assigned by Supervisor
