Requirements

  • Registered Nurse (RN) license
  • Ability to work various shifts and hours as an onsite Patient Educator and Case Manager
  • Competency in emergency operational procedures, including CPR and emergency measures
  • Skill in operating and troubleshooting home dialysis therapy-related equipment
  • Knowledge of company policies, procedures, state and federal regulations for dialysis care
  • Ability to assess patients' home environment and readiness for home dialysis

Responsibilities

  • Provides safe and effective training and oversight of home dialysis therapies (peritoneal and home hemodialysis) to patients and families using state-of-the-art programs and equipment
  • Conducts initial home visits with another staff member and assesses ongoing suitability of home environment for dialysis
  • Educates patients and families on ESRD, Kidney Options Program, renal dialysis, peritoneal/vascular access, and related health conditions
  • Assists in identifying and evaluating candidates for the Home Therapies Program
  • Delivers home therapy support care, assesses patient response to treatments, and communicates issues to physicians
  • Reports adverse events, equipment problems, and coordinates patient transfers to hemodialysis clinic as needed
  • Rotates with staff for coverage, participates in staff training, orientation, and required meetings
  • Ensures quality patient care per company goals, submits CQI information, and participates in CQI/QEP activities and monthly meetings
  • Reviews, transcribes, and enters physician lab orders; handles lab specimen collection, packaging, delivery, and reporting
  • Administers medications as prescribed, maintains records on controlled substances, and initiates emergency procedures
  • Maintains competency in all home dialysis equipment operations and troubleshooting
