Requirements

Must be a Registered Nurse (RN)
Must be available to float to different home programs on Oahu and neighboring islands
Competency in emergency operational procedures, including CPR and emergency measures
Ability to maintain skill level in operation of home dialysis therapy-related equipment, including troubleshooting via telephone
Knowledge of company policies, procedures, standards, and regulations (federal, state, corporate)
Ability to assess patients’ home environment and readiness for home dialysis

Responsibilities

Provides safe and effective training and oversight of home dialysis therapies to patients in compliance with policies, procedures, and regulations
Rotates with other licensed staff to ensure adequate coverage
Participates in staff training, orientation of new staff, and all required staff meetings
Provides outstanding quality patient care per company quality goals and standards
Submits CQI information to Clinical Manager and participates in monthly CQI meetings
Assists in identification and evaluation of candidates for Home Therapies Program
Trains home patients and families on safe operation and maintenance of home therapies equipment
Educates patients and families on ESRD, Kidney Options Program, renal dialysis, access, and related conditions
Assesses patients’ response to home dialysis therapy and communicates issues to physicians
Reports adverse events, equipment problems, and coordinates patient transfers to hemodialysis clinic as needed
Reviews, transcribes, and enters physician lab orders into Medical Information System; handles lab specimen collection, packaging, and delivery
Administers medications as prescribed, follows algorithms, and maintains records on controlled substances
Directs patients in preventative maintenance of home therapies equipment, including water systems
Coordinates with company products division on delivery, operation, and maintenance of home dialysis equipment
Maintains medical records and documentation
