Requirements

  • Functions as part of the Home Therapies dialysis health care team
  • Compliance with company policies, procedures, training, standards, facility policy and procedure manuals, regulations set forth by the Corporation, state, and federal agencies
  • Rotates with other licensed staff to ensure adequate coverage
  • Participates in staff training, orientation of new staff, and all required staff meetings
  • Assists in identification and evaluation of candidates for Home Therapies Program, including assessment of patients’ home environment
  • Assesses patient and family readiness, ability to perform dialysis treatments at home, and home environment suitability
  • Maintains competency with all emergency operational procedures, including initiating CPR and emergency measures
  • Maintains appropriate skill level in operation of all home dialysis therapy-related equipment, including troubleshooting via telephone
  • Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System
  • Follows company policies and government regulations for lab specimen collection, packaging, delivery, and reporting

Responsibilities

  • Provides safe and effective training and oversight of home dialysis therapies delivery to patients
  • Supports company’s Quality Enhancement Program (QEP) and CQI activities, including process improvement for patient satisfaction and quality goals
  • Provides outstanding quality of patient care as defined by company standards; submits CQI information and participates in monthly CQI meetings
  • Trains home patients and families/significant others on safe operation and maintenance of home therapies equipment via formal program
  • Educates patients and family on End Stage Renal Disease (ESRD), Kidney Options Program, renal dialysis, peritoneal or vascular access, home dialysis therapy, and related health conditions
  • Provides safe and effective delivery of home therapy support care to ESRD patients; assesses response to therapy per protocols
  • Communicates patient-related issues to physician; reports adverse events, equipment problems to management and physician with documentation
  • Coordinates patient transfers to hemodialysis clinic when needed
  • Administers medications as prescribed, follows algorithms, documents justification, and maintains controlled substance records
  • Directs patients in preventative maintenance of home therapies equipment, including water systems and analyses
  • Coordinates with company products division on delivery, operation, and maintenance of home therapy dialysis equipment
  • Admits new patients and documents all relevant data, medical records, and patient-related information
