Responsibilities

  • Directs Patient Care Technicians in providing safe and effective chronic hemodialysis therapy in compliance with facility policies, procedures, and corporate/state/federal regulations
  • Delegates tasks to direct patient care staff including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants
  • Assesses daily patient care needs and develops appropriate patient care assignments
  • Routinely monitors patient care staff for appropriate techniques and adherence to facility policies and procedures
  • Participates in staff training, orientation of new staff, and all required staff meetings
  • Ensures educational needs of patients and families are met regarding End Stage Renal Disease (ESRD), renal disease, vascular access, dialysis therapy, related health conditions, diet/fluid/medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, laboratory values, and dialysis therapy compliance
  • Provides safe and effective delivery of care to patients with ESRD, including accurate implementation of treatment prescriptions (e.g., Sodium modeling, Ultrafiltration modeling)
  • Assesses patients’ response to hemodialysis therapy, makes adjustments/modifications to treatment plans as indicated by physician, and communicates problems/concerns to Team Leader or physician
  • Identifies and communicates patient-related issues to Team Leader or physician
  • Initiates Initial and Annual Nursing Assessments, ongoing evaluation, and documentation of patient care needs per FMC policies
  • Actively participates in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatments per established procedures
  • Takes appropriate interventions for changes in patient adequacy status and troubleshoots access flow issues (e.g., OLC/AMP yellow lights)
  • Provides, supervises (if applicable), and monitors hemodialysis access care per established procedures
  • Implements, administers, monitors, and documents patient responses to prescribed interdialytic transfusions, notifying physician and blood supplier of adverse reactions
  • Ensures accurate and complete documentation by Patient Care Technicians on Hemodialysis Treatment Sheets
  • Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System
  • Ensures appropriate preparation of lab work
