Requirements

  • Must be a Registered Nurse (RN)
  • Must be able to float between Kona and Kamuela as needed
  • Competency in emergency operational procedures, including CPR and emergency measures
  • Appropriate skill level in the operation of all home dialysis therapy-related equipment, including troubleshooting via telephone
  • Ability to maintain records on controlled substances as required by law

Responsibilities

  • Provides safe and effective training and oversight of home dialysis therapies to patients in compliance with policies, procedures, and regulations
  • Rotates with other licensed staff to ensure adequate coverage
  • Participates in staff training, orientation of new staff, and all required staff meetings
  • Provides outstanding quality of patient care per company quality goals and standards
  • Submits CQI information to Clinical Manager and participates in all monthly CQI meetings
  • Assists in identifying and evaluating candidates for the Home Therapies Program, including assessing patients’ home environment
  • Trains home patients and families on safe operation and maintenance of home therapies equipment through formal training program
  • Educates patients and families on ESRD, Kidney Options Program, renal dialysis, access, and related health conditions
  • Assesses patient and family readiness, ability to perform home dialysis, and home environment suitability
  • Provides safe and effective delivery of home therapy support care to ESRD patients
  • Assesses patients’ response to home dialysis therapy per protocols
  • Communicates patient issues to physicians and reports adverse events or equipment problems to management and physicians with proper documentation
  • Coordinates patient transfers to hemodialysis clinic when needed
  • Reviews, transcribes, and enters physician lab orders into the Medical Information System
  • Follows policies and regulations for lab specimen collection, packaging, delivery, and reporting
  • Administers medications as prescribed, following algorithms and documenting justification
  • Directs patients in preventative maintenance of home therapies equipment, including water systems
  • Coordinates with company products division on delivery, operation, and maintenance of home dialysis equipment
  • Admits all new patients
  • Maintains competency with emergency procedures and initiates CPR/emergency measures as needed
