Requirements

  • Registered Nurse (RN) license
  • Entry-level designation into Clinical Advancement Program (CAP 1); required to complete CAP requirements to advance
  • Ability to perform physical demands: heavy lifting, moving equipment/patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. with assistance, lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Work in a health care environment characteristic of heavy patient care

Responsibilities

  • Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Assess and manage patients’ response to home dialysis training and treatment therapy following prescribed protocols; communicate patient issues to physician as needed
  • Perform ongoing systematic collection and analysis of dialysis data, document in medical record, adjust/modify treatment plan, and notify Supervisor or physician
  • Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education
  • Direct and provide, in collaboration with patient/home care partner and staff, safe and effective delivery of dialysis therapy
  • Administer medications as prescribed or per approved algorithms; document justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource for health care team; participate in staff training and orientation of new staff
  • Ensure correct laboratory collection, processing, and shipping; reschedule missed/insufficient collections
  • Identify expected outcomes, document/update nursing assessment and plan of care via collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform treatments
  • Train Home Dialysis patients/Home Partners on safe, effective operation and maintenance of equipment/supplies through formal Home Dialysis Training Program
  • Participate in decision-making, teaching, leadership, and quality improvement activities to enhance patient care and facility operations
  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, nursing standards, state/federal regulations, under direction of Supervisor with guidance from Educator/Preceptor or in collaboration with another RN
  • Perform all other duties as assigned by Supervisor
