Requirements

  • Registered Nurse (RN) license
  • Entry-level designation into the Clinical Advancement Program (CAP RN 1)
  • Complete requirements to advance to the next CAP level within 24 months from date of hire
  • Perform duties under direction of Program Manager with guidance from Educator, Preceptor, or Home Therapy CAP RN 2-5 for first 6 months

Responsibilities

  • Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment
  • Conduct initial home visits with another staff member present
  • Function as onsite Patient Educator and Case Manager with various shifts and work hours
  • Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Perform ongoing, systematic review and analysis of dialysis data for Home Therapy patients, document in medical record, adjust treatment plan, and notify Supervisor or Physician as needed
  • Recognize and report aspects of patient status varying from normal, collaborating with health team members
  • Assess patient care needs and collaborate with direct and ancillary staff to deliver safe and effective chronic dialysis therapy
  • Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
  • Ensure correct patient laboratory collection, processing, shipping, and reschedule missed collections
  • Ensure physician orders are entered and acknowledged in the Medical Record
  • Initiate or assist with emergency response measures
  • Delegate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians) and monitor adherence to policy and procedures
  • Promote infection control, equipment, and environmental safety
  • Assess, collaborate, and document patient/family learning needs for initial and ongoing education
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate's home environment, partner/family readiness, and ability to perform treatments
  • Admit new patients to Home Therapy per policy and procedure
  • Train Home Dialysis patients and/or Home Partners on safe operation and maintenance of equipment and supplies through formal training program
  • Assess and manage patients' response to home dialysis training and therapy per protocols
  • Communicate patient status and progress
