Requirements

  • Registered professional nurse (RN) designation, entry level into Clinical Advancement Program (CAP RN 1)
  • Accountability for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities
  • Completion of requirements to advance to next CAP level within 24 months from hire date
  • Performance of all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
  • Performs essential functions under direction of Program Manager with guidance from Educator, Preceptor, or higher-level CAP RN for first 6 months

Responsibilities

  • Performs ongoing, systematic review and analysis of dialysis data for Home Therapy patients, documents in medical record, adjusts treatment plan, and notifies Supervisor or Physician as needed
  • Recognizes and reports aspects of patient status varying from normal, collaborates with health team members
  • Assesses assigned patient care needs and collaborates with direct and ancillary staff
  • Directs and provides safe and effective delivery of chronic dialysis therapy to assigned patients
  • Administers medications as prescribed or per approved algorithms, documents justification and effectiveness
  • Ensures correct patient laboratory collection, processing, shipping, and reschedules missed collections
  • Ensures physician orders are entered and acknowledged in the Medical Record
  • Initiates or assists with emergency response measures
  • Delegates tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians) and monitors adherence to policy
  • Promotes infection control, equipment, and environmental safety
  • Assesses, collaborates, and documents patient/family learning needs, provides initial and ongoing education
  • Assists in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Performs assessment of Home Dialysis candidate's home environment, partner/family readiness, and admits new patients per policy
  • Trains Home Dialysis patients/Home Partners on equipment and supplies through formal training program
  • Assesses and manages patient response to home dialysis training and therapy per protocols, communicates issues to physician
  • Identifies expected outcomes, documents and updates nursing assessment and plan of care via Interdisciplinary Team
  • Ensures patient awareness of transplant and treatment modality options
  • Makes referrals to Social Worker and Registered Dietitian as appropriate
  • Checks availability of adequate emergency equipment and supplies
