Registered Nurse (RN) license
Entry-level designation into the Clinical Advancement Program (CAP); required to complete CAP requirements to advance
Ability to perform physically demanding tasks, including heavy lifting and moving of patients
Works primarily in clinic setting with various shifts and work hours; conducts initial home visits with another staff member present

Responsibilities

Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment
Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, make adjustments to treatment plan, and notify Supervisor or physician as needed
Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
Direct and provide, in collaboration with patient, home care partner, and staff, all aspects of safe and effective delivery of dialysis therapy
Administer medications as prescribed or per approved algorithms, and document justification and effectiveness
Initiate or assist with emergency response measures
Serve as a resource for health care team, participate in staff training and orientation of new staff
Ensure correct laboratory collection, processing, shipping, and reschedule missed collections
Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform treatments
Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of all Home Dialysis equipment and supplies through organized training program
Participate in decision-making, teaching, leadership functions, and quality improvement activities
Perform all duties under direction of Supervisor with guidance from Educator, Preceptor, or in collaboration with another RN
Perform all other duties as assigned by Supervisor
