Requirements

  • Must reside in Mercer or Middlesex Counties, NJ
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time within Mercer and Middlesex Counties (mileage reimbursed per company policy)
  • Must be comfortable completing field-based assessments for all socioeconomic statuses and settings
  • Must hold an active unrestricted Registered Nurse license in the state of New Jersey
  • 2+ years clinical practice experience (e.g., hospital setting, home health, or ambulatory care)
  • Successful completion of the NJ Choice certification (exam score of 80% or higher, state training modules, and field mentoring; contingency of employment)
  • Attention to detail and be a team player
  • Computer knowledge and experience handling medical records
  • Must be able to work Monday - Friday, 8:00am - 5:00pm EST
  • Minimum Associates Degree in Nursing (Bachelor's in Nursing preferred)

Responsibilities

  • Conduct face-to-face assessments with members to evaluate medical needs and facilitate overall wellness and access to services (e.g., adult/pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program, MLTSS enrollment)
  • Complete a minimum of 20 assessments per week (may be more), considering information from various sources, co-morbid conditions, multiple diagnoses, functionality, work capacity, and restrictions/limitations
  • Use a holistic approach to assess the need for referrals to clinical resources for functionality determination
  • Consult with supervisor and team to overcome barriers in meeting goals and objectives
  • Utilize case management processes in compliance with regulatory and company policies/procedures
  • Use interviewing skills to maximize member engagement and discern health status/needs
  • Mentor new hires once proficiency in the role is attained
  • Operate at full capacity in the role by month four of employment and training
