Requirements

  • Must possess reliable transportation and be willing and able to travel up to 75% of the time within Ocean and Burlington Counties
  • Must be comfortable completing field-based assessments for all socioeconomic status and settings
  • Must hold an active unrestricted Registered Nurse license in the state of New Jersey
  • 2+ years clinical practice experience (e.g., hospital setting, alternative care setting such as home health or ambulatory care) required
  • Successful completion of the NJ Choice certification (exam score of 80% or higher, state training modules, and field mentoring component) is a contingency of employment
  • Must be able to work Monday - Friday, 8:00am - 5:00pm EST
  • Have reliable phone and hard-wired internet service for work-from-home setup
  • Work space must be secure and private to maintain HIPAA compliance
  • Proficient in Multisystem Navigation

Responsibilities

  • Conduct face-to-face assessments with members to evaluate medical needs and facilitate overall wellness
  • Address requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program, and MLTSS program enrollment
  • Complete a minimum of 20 assessments per week (may be more), considering information from various sources, co-morbid conditions, multiple diagnoses, functionality, work capacity, and restrictions/limitations
  • Use a holistic approach to assess the need for referrals to clinical resources
  • Consult with supervisor and team to overcome barriers in meeting goals and objectives
  • Utilize case management processes in compliance with regulatory and company policies and procedures
  • Use interviewing skills to ensure maximum member engagement and discern health status and needs
  • Mentor new hires on the team once proficiency in the role is attained
