Registered Nurse Assessor - Field In Ocean and Burlington Counties NJ at CVS Health

The role is primarily work-from-home out of your residence in NJ with reliable phone and hard-wired internet, but it is field-based requiring regular work in multiple locations including member homes, nursing homes, rehabilitation centers, and community locations in Ocean and Burlington Counties.

Work schedules are Monday-Friday 8-5 EST, determined by the leader based on business needs, with colleagues required to be reachable via phone, videoconference, and email during their 8-hour workday.

The RN Field Assessor conducts face-to-face assessments with members to evaluate medical needs, facilitate wellness, and help obtain services; completes a minimum of 20 assessments per week; uses holistic approaches, interviewing skills, and case management processes while mentoring new hires.

Candidates must be a Registered Nurse (RN) able to perform field assessments in Ocean and Burlington Counties NJ, work from a secure home office with reliable hard-wired internet and phone, and have skills in holistic assessment, interviewing, case management, and mentoring.

The role requires a minimum of 20 assessments per week (may be more), consulting with supervisors and team members to overcome barriers, and mentoring new hires once proficient.
