Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education on diet's impact on treatment
  • Deliver detailed education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
  • Calculate diet prescriptions according to standards of practice
  • Complete progress notes on chronic dialysis patients and maintain accurate documentation per FMCNA and regulatory guidelines
  • Provide ongoing nutrition assessment, counseling, and monitoring to achieve effective nutritional status; identify and address malnutrition risks
  • Review patient albumin levels and body weight monthly; implement and adjust plan of care with patient/family involvement
  • Monitor adherence/response to nutrition therapy, document barriers to goals, and collaborate on Urea Kinetic Modeling, mineral metabolism, anemia, psychosocial status, vascular access, and other areas
  • Adhere to FMCNA algorithms; report on nutrition QAI results and participate in QAI program
  • Collaborate on Pre-ESRD education and coordinate with Case Manager for patient education and physician referrals for supplements
  • Review nutrition-related lab results; counsel patients/caregivers, recommend treatment changes, and communicate with physicians, staff, and external facilities
  • Participate in pertinent staff meetings (e.g., general staff, quality improvement)
