Responsibilities

Conducts nutritional assessments and participates in comprehensive assessments with the interdisciplinary team
Provides detailed nutrition education to patients, families, and caregivers regarding diet, kidney disease, and its impact on treatment
Develops individualized comprehensive plans of care, specifying necessary services to meet patient needs
Calculates diet prescriptions according to established standards
Completes progress notes on chronic dialysis patients and maintains accurate medical records
Provides ongoing nutrition assessment and outcome-oriented counseling to help patients achieve nutritional goals
Identifies malnourished patients and collaborates with the team to implement interventions
Reviews patient albumin levels and body weight monthly
Works with the interdisciplinary team on Urea Kinetic Modeling recommendations and management of mineral metabolism, anemia, and renal bone disease
Monitors patient adherence to nutrition therapy, adjusts treatment as needed, and documents reasons for non-achievement of goals
