Responsibilities

  • Calculates diet prescriptions according to established standards
  • Completes progress notes on chronic dialysis patients and maintains accurate medical records
  • Provides ongoing nutrition assessment and outcome-oriented counseling to help patients achieve nutritional goals
  • Identifies malnourished patients and collaborates with the team to implement interventions
  • Reviews patient albumin levels and body weight monthly
  • Works with the interdisciplinary team on Urea Kinetic Modeling recommendations and management of mineral metabolism, anemia, and renal bone disease
  • Monitors patient adherence to nutrition therapy, adjusts treatment as needed, and documents reasons for non-achievement of goals
