Registered Dietitian credentials
Knowledge of diet and kidney disease relationship and impact on treatment
Ability to assess patient's learning needs and provide appropriate education
Skills in nutritional assessment and calculating diet prescriptions according to standards of practice
Proficiency in completing progress notes and maintaining accurate documentation per FMCNA policies, Federal, and state regulations
Competence in monitoring lab results (e.g., albumin, body weight, nutrition-related parameters) and adjusting nutrition therapy
Ability to collaborate with interdisciplinary teams on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Experience in process improvement, QEP, QAI activities, and reporting nutrition QAI results
Capability to identify malnourished patients or those at risk and recommend interventions

Responsibilities

Assess patient’s knowledge of diet and kidney disease and provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and development of individualized plans of care per Federal and state guidelines
Calculate diet prescriptions and provide ongoing nutrition assessment and counseling to achieve effective nutritional status
Review patient’s albumin level and body weight monthly, implement and monitor plan of care with patient/family/caregiver involvement
Monitor adherence/response to nutrition therapy, adjust as needed, document barriers, and address issues impacting goals
Collaborate with interdisciplinary team on Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia/iron, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control; adhere to FMCNA algorithms and policies
Report nutrition QAI results and participate in interdisciplinary QAI program and staff meetings
Coordinate Pre-ESRD education, work with Case Manager on patient education and referrals for nutritional supplements
Review nutrition-related lab results, counsel patient/caregiver, recommend treatment changes, and communicate with physicians, facility staff, and care providers in nursing facilities
