Requirements

  • Registered Dietitian licensure
  • Knowledge of diet and kidney disease relationship and impact on treatment
  • Ability to assess patient's nutritional status, learning needs, adequacy measures, fluid status, and nutrition-related lab parameters
  • Skills in calculating diet prescriptions according to standards of practice
  • Proficiency in completing nutritional assessments, progress notes, and maintaining documentation per FMCNA policies, Federal, and state regulations
  • Competence in urea kinetic modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Ability to collaborate with interdisciplinary teams, physicians, facility staff, and case managers
  • Participation in QAI, QEP, process improvement, and staff meetings

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education to patient/family/caregiver on nutritional status, adequacy, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development per Federal/state guidelines
  • Calculate diet prescriptions and provide ongoing nutrition assessment, counseling, and therapy adjustments to achieve effective nutritional status
  • Identify malnourished or at-risk patients; collaborate on interventions with interdisciplinary team
  • Review monthly albumin levels and body weight; implement, monitor, and adjust plan of care with patient/family/caregiver involvement
  • Document all information accurately in medical records per guidelines
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Report nutrition QAI results and participate in interdisciplinary QAI program
  • Coordinate Pre-ESRD education and refer for nutritional supplements as appropriate
  • Review nutrition-related lab results; counsel patient/caregiver, recommend treatment changes, and communicate with physicians, staff, and care providers in facilities
