Requirements

  • Registered Dietitian licensure
  • Knowledge of diet and kidney disease relationship and impact on treatment
  • Ability to assess patient nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Skills in calculating diet prescriptions according to standards of practice
  • Proficiency in completing nutritional assessments and progress notes per FMCNA guidelines, federal, and state regulations
  • Competence in Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, and sodium/volume control management
  • Ability to collaborate with interdisciplinary teams on comprehensive plans of care, QAI/QEP activities, and process improvements
  • Experience in patient/family/caregiver education, counseling, and identifying malnutrition risks/interventions

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education on diet's impact on treatment
  • Deliver detailed education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and plan of care development
  • Calculate diet prescriptions and complete progress notes for chronic dialysis patients per guidelines
  • Maintain accurate documentation in medical records per FMCNA policies and regulations
  • Provide ongoing nutrition assessment, counseling, and therapy adjustments to achieve effective nutritional status
  • Identify and address malnourished/at-risk patients; review albumin levels and body weight monthly; implement care plans
  • Monitor adherence/response to nutrition therapy; document barriers and adjust plans accordingly
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adhere to FMCNA algorithms; report on nutrition QAI results; participate in QAI programs and staff meetings
  • Coordinate Pre-ESRD education and work with Case Manager on patient education, nutritional supplements referrals
  • Review nutrition-related lab results; counsel patients/caregivers; recommend treatment changes; communicate with physicians, staff, and external facilities
