Requirements

  • Registered Dietitian credentials
  • Knowledge of diet and kidney disease relationship and impact on treatment
  • Ability to assess patient's learning needs and provide appropriate education
  • Skills in nutritional assessment and calculating diet prescriptions per standards of practice
  • Proficiency in completing progress notes and maintaining accurate medical record documentation per FMCNA policies, Federal, and state regulations
  • Competence in monitoring lab results (e.g., albumin, body weight, nutrition-related parameters) and adjusting nutrition therapy
  • Ability to collaborate with interdisciplinary teams on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adherence to FMCNA algorithms and participation in QAI/QEP activities

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease and provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and development of individualized plans of care per Federal/state guidelines
  • Provide ongoing nutrition assessment, outcome-oriented counseling, and implement/adjust plans of care to achieve effective nutritional status, involving patient/family/caregiver
  • Identify malnourished or at-risk patients and collaborate on interventions/resources
  • Review patient’s albumin level and body weight monthly; monitor adherence/response to therapy and document barriers to goals
  • Collaborate with interdisciplinary team on Kt/V recommendations, disease management, and Pre-ESRD education
  • Review nutrition-related lab results, counsel patient/caregiver, recommend treatment changes, and communicate with physicians, facility staff, and external providers (e.g., nursing facilities)
  • Participate in staff meetings, report on nutrition QAI results, and engage in process improvement activities to support patient goals
