Experience with interdisciplinary team collaboration, including development of individualized care plans per Federal and state guidelines
Skills in documentation per FMCNA policies, Federal/state regulations, and medical record standards
Familiarity with Quality Enhancement Program (QEP), Quality Assessment and Performance Improvement (QAI), and process improvement activities
Ability to identify malnutrition risks and implement interventions

Responsibilities

Assess patient’s knowledge of diet and kidney disease; provide education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
Calculate diet prescriptions according to standards of practice
Complete progress notes on chronic dialysis patients and maintain accurate medical record documentation
Provide ongoing nutrition assessment, counseling, and monitoring; adjust therapy as needed and document barriers to goals
Review patient’s albumin level and body weight monthly; implement plan of care with patient/family/caregiver involvement
Identify malnourished or at-risk patients and collaborate on interventions/resources
Collaborate with interdisciplinary team on Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Adhere to FMCNA algorithms, report on nutrition QAI results, and participate in QAI program
Coordinate Pre-ESRD education and work with Case Manager for patient education, nutritional supplements referrals
Review nutrition-related lab results, counsel patient/caregiver, recommend treatment changes, and communicate with physicians, facility staff, and external providers (e.g., nursing facilities)
Participate in pertinent staff meetings (e.g., general staff, quality improvement)
