Registered Dietitian credentials
Knowledge of diet, kidney disease, and its impact on treatment processes
Ability to assess patient's nutritional status, knowledge, and learning needs
Proficiency in calculating diet prescriptions according to standards of practice
Skills in nutritional assessment, education, and counseling
Ability to interpret nutrition-related lab parameters (e.g., albumin, body weight, Kt/V, mineral metabolism)
Familiarity with Federal and state regulations, FMCNA policies, and guidelines for documentation and care planning
Experience with interdisciplinary collaboration and process improvement activities (QEP, QAI)

Responsibilities

Assess patient’s knowledge of diet and kidney disease; provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and lab parameters
Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
Calculate diet prescriptions and complete progress notes on chronic dialysis patients per FMCNA guidelines
Maintain accurate documentation in medical records according to policies and regulations
Provide ongoing nutrition assessment, counseling, and monitoring to achieve effective nutritional status; identify and address malnutrition risks
Review patient’s albumin level and body weight monthly; implement and adjust plan of care with patient/family involvement
Monitor adherence to nutrition therapy, adjust as needed, and document barriers to goals
Collaborate with interdisciplinary team on Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Adhere to FMCNA algorithms; report on nutrition QAI results and participate in QAI program
Coordinate Pre-ESRD education and work with Case Manager for patient education and referrals for nutritional supplements
Review nutrition-related lab results; counsel patient/caregiver, recommend treatment changes, and communicate with physicians, facility staff, and external providers (e.g., hospitals, nursing facilities)
Participate in pertinent staff meetings (e.g., general staff, quality improvement)
