  • Ability to collaborate with interdisciplinary teams on care plans, QAI/QEP activities, process improvements, and patient-specific goals
  • Competence in Urea Kinetic Modeling, renal bone disease management, and identifying malnutrition risks

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
  • Calculate diet prescriptions and complete progress notes on chronic dialysis patients per FMCNA guidelines
  • Provide ongoing nutrition assessment, counseling, and therapy adjustments to achieve effective nutritional status; identify and address malnutrition risks
  • Monitor adherence/response to nutrition therapy, review monthly albumin and body weight, and implement care plans with patient/family involvement
  • Collaborate with interdisciplinary team on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adhere to FMCNA algorithms/policies; report on nutrition QAI results and participate in QAI program
  • Coordinate Pre-ESRD education, work with Case Manager on care plans, and refer for nutritional supplements as needed
  • Review nutrition-related lab results; counsel patient/caregiver, recommend treatment changes, and communicate with physicians, facility staff, and external providers (e.g., hospitals, nursing facilities)
  • Participate in pertinent staff meetings (e.g., general staff, quality improvement)
