Requirements

  • Registered Dietitian credentials
  • Knowledge of diet and kidney disease relationship and impact on treatment
  • Ability to assess patient's learning needs and provide appropriate education
  • Skills in nutritional assessment and calculating diet prescriptions per standards of practice
  • Proficiency in completing progress notes and maintaining accurate medical record documentation per FMCNA policies, Federal, and state regulations
  • Capability to monitor patient adherence, adjust nutrition therapy, and implement plans of care
  • Experience with Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Ability to collaborate with interdisciplinary teams, including physicians, case managers, and facility staff
  • Participation in QAI, QEP, process improvement, and staff meetings

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease and provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and development of individualized plans of care per Federal and state guidelines
  • Calculate diet prescriptions and provide ongoing nutrition assessment and counseling to achieve effective nutritional status
  • Identify malnourished or at-risk patients, collaborate on interventions, review monthly albumin levels and body weight, and implement plans of care with patient/family/caregiver involvement
  • Monitor adherence/response to nutrition therapy, adjust as needed, document barriers to goals, and address issues impacting achievement
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia/iron, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control; adhere to FMCNA algorithms and policies
  • Report on nutrition QAI results, participate in interdisciplinary QAI program, and coordinate Pre-ESRD education with team/Case Manager
  • Review nutrition-related lab results, counsel patient/caregiver, recommend treatment changes, and communicate with physicians, facility staff, and nursing facilities
  • Participate in pertinent staff meetings (e.g., general staff, quality improvement)
