Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
  • Calculate diet prescriptions and complete progress notes on chronic dialysis patients per FMCNA guidelines
  • Provide ongoing nutrition assessment, counseling, and monitoring; identify and address malnutrition risks; review albumin and body weight monthly
  • Implement, monitor, and adjust nutrition therapy; document barriers to goals and collaborate on interventions
  • Collaborate with interdisciplinary team on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adhere to FMCNA algorithms and policies; report on nutrition QAI results and participate in QAI programs
  • Coordinate Pre-ESRD education and work with Case Manager on patient education, nutritional supplements, and physician referrals
  • Review nutrition-related lab results; counsel patients/caregivers; recommend and communicate treatment changes to physicians, facility staff, and external providers (e.g., hospitals, nursing facilities)
  • Participate in pertinent staff meetings (e.g., general staff, quality improvement)
