Responsibilities
- Inhoudelijk aanspreekpunt voor de teams
- Meekijken bij complexe casussen
- Ondersteunen en coachen van persoonlijk begeleiders op methodisch en planmatig handelen
- Vormen van het voorportaal voor de gedragsdeskundige bij cliënten waar Odion behandelverantwoordelijkheid heeft
- Ondersteunen van persoonlijk begeleiders om de juiste koers te bepalen
- Meewerken in de ADL en somatische zorg om binding te houden met cliënten en team
- Invulling geven aan ontwikkelopgaven: Samen zorgen (samenwerking met familie en naasten), Meedoen (betrekken buurt en ontwikkelen dagbesteding), Zorgtechnologie (inzetten en ontwikkelen)
- Ondersteunen bij het aanvragen van indicaties
- Samenwerken met teamleider en gedragsdeskundige voor verbinding tussen visie en praktijk
