Requirements

Candidates must have 1+ years of experience in a healthcare, clinical, or community services setting with a focus on care coordination and resource navigation. A minimum of 1+ years of demonstrated experience in managing complex referral processes is also required. Experience with Athena is preferred. Candidates must also provide proof of COVID-19 vaccination or an approved exemption, and complete medical clearance requirements including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved accommodation.

Responsibilities

The Referral Coordinator will manage the referral process end-to-end, liaising with and completing paperwork for external care providers, facilities, health insurance benefits programs, and government/community support options. This role involves communicating with members and external agencies regarding referrals, initiating and managing referrals based on member needs, and coordinating referrals with internal and external stakeholders. The coordinator will collaborate with external PCPs to facilitate specialist and home health agency referrals. Additionally, they will leverage care facilitation tools, electronic health records, and scheduling platforms to document interactions, gather data, organize information, manage tasks, and maintain effective communication.
