Responsibilities

Provide behavioral healthcare management to patients, monitoring and managing care using a holistic approach and array of treatment modalities through recovery phases
Co-manage a panel of complex patients through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning
Develop individualized treatment plans early in engagement and stabilization, addressing psychosocial and medical needs limiting recovery
Provide continuous, regular consultations in person and occasionally virtually as patients navigate recovery
Liaise with patient care teams and collaborate closely with behavioral health providers, primary care providers, peer specialists, and community partners for integrated care
Act as a patient advocate, helping members navigate health and social services systems, addressing problems, answering questions, gathering/providing information and advice, and connecting to resources/community
Initiate, establish, and maintain trust-based relationships with patients and families
Address medical needs to mitigate negative impacts of social determinants of health
