Responsibilities

  • Provide behavioral healthcare management to patients, monitoring and managing care using a holistic approach and array of treatment modalities through recovery phases
  • Co-manage a panel of complex patients through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning
  • Develop individualized treatment plans early in engagement and stabilization, addressing psychosocial and medical needs limiting recovery
  • Provide continuous, regular consultations in person and occasionally virtually as patients navigate recovery
  • Liaise with patient care teams and collaborate closely with behavioral health providers, primary care providers, peer specialists, and community partners for integrated care
  • Act as a patient advocate, helping members navigate health and social services systems, addressing problems, answering questions, gathering/providing information and advice, and connecting to resources/community
  • Initiate, establish, and maintain trust-based relationships with patients and families
  • Address medical needs to mitigate negative impacts of social determinants of health
