Requirements

  • Pass a Criminal Background Check (CBC) through Oregon's Department of Human Services

Responsibilities

  • Administer National Outcomes Measure Scale (NOMS) tools to eligible clients, participate in data collection and reporting, and follow established format, training, and procedures
  • Coordinate client schedules, including arranging appointments, confirmation calls, and follow-up for no shows; explain appointment purposes in clear, simple language
  • Collect data to support client/patient goals and satisfy grant requirements; share with leadership as necessary
  • Adhere to compliance, required laws, trainings, and safety regulations (e.g., mandatory abuse reporting, HIPAA, fire regulations, agency policies)
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental/agency meetings as indicated
  • Ensure compliance of documentation and maintain clinical records in the Electronic Health Record System (EHR) per agency, local, state, and federal regulations
  • Complete all annual employee training requirements for ethical work, professional development, and compliance
  • Support Wellness Program initiatives and the resource room; opportunities to perform Rapid Engagement Screenings
  • Perform other job-related duties as assigned
