Requirements

  • Graduate of an accredited School of Nursing and Nurse Practitioner Program (or meet standards under California Code of Regulations Section 1484)
  • Current RN and NP licensure in the state
  • CPR certification required
  • Master’s degree with at least one (1) year NP experience and one (1) year hospice or palliative care experience
  • Certification by an approved advanced practice nursing certifying body; specialty certification (e.g., CHPN, Pediatrics, Geriatrics) preferred
  • Strong assessment, communication, problem-solving, and nursing skills; able to meet physical demands of patient care
  • Valid driver’s license, reliable insured vehicle, and meet organization driving requirements

Responsibilities

  • Deliver comprehensive hospice services in accordance with the Nurse Practice Act and under a collaborating physician’s supervision
  • Conduct patient evaluations, health assessments, diagnosis, and management of care plans, including prescribing medications and treatments
  • Perform physical exams, face-to-face assessments, and hospice eligibility evaluations in consultation with the interdisciplinary team
  • Collaborate with patients, families, and healthcare practitioners to coordinate care
  • Utilize community resources to develop and direct services supporting patients and families at home and in the community
