Requirements

  • Active license as a Physical Therapist in the state of practice (Upon Hire)
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
  • Ability to work primarily days with rotating weekends (1 weekend per month) and holidays (2 holidays per year)

Responsibilities

  • Evaluate assigned patients, gathering data from patient records, interviews, observation, and team members
  • Develop and implement individualized treatment programs for patients, review plan of care with patient, and instruct patient, family, caregivers in home programs or exercises
  • Use selected activities/tasks to promote health, achieve treatment goals, and modify interventions based on ongoing evaluation
  • Provide direct therapy services in various functional settings, tailored to patient's physical, emotional, and intellectual capabilities, coordinated with rehabilitation, medical, and nursing plans
  • Prepare documentation including evaluation results, individualized treatment plans, progress notes, and other reports
  • Communicate effectively with patient, family, physician, and interdisciplinary team members regarding status, progress, and needs; attend care conferences
  • Establish rapport and work cooperatively with interdisciplinary team members (including family/physician)
  • Enhance professional growth through educational programs, literature, in-services, workshops, etc
  • Oversee physical therapy assistants, techs, and students to ensure safe patient care and smooth operations
  • Maintain knowledge of proper use of equipment for patient care
  • Interview, hire, develop, train, retain employees; plan, assign, lead work; appraise performance; reward and coach employees; address complaints and resolve problems
