Active license as a Physical Therapist in the state of practice (Upon Hire)
Visual acuity adequate to perform position duties
Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
Ability to work primarily days with rotating weekends (1 weekend per month) and holidays (2 holidays per year)

Responsibilities

Evaluate assigned patients, gathering data from patient records, interviews, observation, and team members
Develop and implement individualized treatment programs for patients, review plan of care with patient, and instruct patient, family, caregivers in home programs or exercises
Use selected activities/tasks to promote health, achieve treatment goals, and modify interventions based on ongoing evaluation
Provide direct therapy services in various functional settings, tailored to patient's physical, emotional, and intellectual capabilities, coordinated with rehabilitation, medical, and nursing plans
Prepare documentation including evaluation results, individualized treatment plans, progress notes, and other reports
Communicate effectively with patient, family, physician, and interdisciplinary team members regarding status, progress, and needs; attend care conferences
Establish rapport and work cooperatively with interdisciplinary team members (including family/physician)
Enhance professional growth through educational programs, literature, in-services, workshops, etc
Oversee physical therapy assistants, techs, and students to ensure safe patient care and smooth operations
Maintain knowledge of proper use of equipment for patient care
Interview, hire, develop, train, retain employees; plan, assign, lead work; appraise performance; reward and coach employees; address complaints and resolve problems
