Active Georgia RN license
PALS & BLS required
EMS (EMT/P) experience preferred
Bachelor's degree in nursing (BSN) required
Minimum 4+ years of pediatric nursing experience
1–2 years in pediatric ED, urgent care, or inpatient preferred
Valid driver's license, reliable transportation, and good driving record
Bilingual in Spanish (preferred)
Passionate and committed to reimagining pediatric health care for children with complex medical conditions

Responsibilities

Travel to members’ homes or community settings to deliver episodic, longitudinal, and urgent care support
Build trusted relationships with patients and families, ensuring compassionate, coordinated care
Collaborate as part of an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists to provide seamless support across medical, behavioral, and social needs
Conduct and document in-home assessments, perform procedures (ear irrigation, point of care testing, IVs and lab draws, wound care, medication administration), provide patient and family education and follow-up care using the Imagine Pediatrics care platform
Perform as an acute care registered nurse a few days per week to conduct virtual nursing, patient assessments, and manage urgent pediatric needs using established protocols
Conduct documentation in the electronic medical records (Athena) and utilize the Imagine Pediatrics proprietary application to communicate with families
