Requirements

  • Functions under direct supervision of a licensed Home Therapies registered nurse (HT RN)
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Ability to reinforce home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law
  • Skills in obtaining vital signs, weights, and patient information
  • Capability to build and maintain communication/collaboration with patients, care partners, staff, and providers
  • Knowledge of electronic home treatment/medication reconciliation and laboratory specimen handling/shipping
  • Ability to set up, tear down, program, test, disinfect, and troubleshoot HHD machines
  • Skills in evaluating dialysis access, initiating HHD treatment, monitoring/documenting patient status, vital signs, and machine data
  • Performance of safety checks per CMS and facility policy

Responsibilities

  • Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law
  • Identifies and welcomes patients, obtains vital signs, weights, and gathers information for Clinic Visit Intake form
  • Builds and maintains effective communication and collaboration with patients, care partners, HT RN, staff, providers, and team members
  • Obtains treatment records, reviews for completion, and reports missing/abnormal data to HT RN
  • Assists with electronic home treatment/medication/oral nutritional supplement reconciliation and reports missing data to HT RN
  • Assists HT RNs with home visits and provides home support as applicable/permissible
  • Prepares supplies for laboratory testing, prepares specimens for shipping, and assists with transplant-related specimens/education
  • Completes opening/closing daily chores, disinfects equipment (machines, chairs, tables), and prepares rooms for training/treatment
  • Stocks rooms with supplies, manages supplies to minimize waste, schedules appointments, and contacts patients/caregivers
  • Supports patient registration, connected health, supplies/inventory ordering, logs/tracking, and clinic/office supplies ordering
  • Participates in medical records, infection control, audits, staff huddles/meetings, and collects documentation for patient charts
  • Performs clerical duties (answering phones, faxing, email, provider contacts)
  • Sets up, tears down, programs, tests, disinfects HHD machine, troubleshoots equipment/connectivity
  • Prepares dialysis equipment/supplies for HHD treatment
  • Evaluates dialysis access before treatment, documents findings, reports abnormalities to HT RN
  • Initiates HHD treatment, monitors/documents patient status, vital signs, machine data
  • Performs and documents safety checks during treatment per CMS/facility policy
  • Supports organization's Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements
