Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and corporation, state, and federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to perform setup and operation of hemodialysis machines
  • Ability to calculate and enter dialysis machine programming for Ultrafiltration (UF) goal, treatment time, and UF modeling
  • Ability to ensure safety alarms are enabled, including Narrow Venous Limits (NVL) 160
  • Ability to monitor patients, evaluate vascular access, obtain vital signs, weights, and perform pre/post dialysis evaluations
  • Ability to document accurately on Hemodialysis Treatment Sheet and progress notes
  • Ability to observe patients, conduct machine safety checks, and report changes to RN or supervisor
  • Ability to assist in training direct patient care staff

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin as allowed
  • Obtain pre and post treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis treatment per prescribed orders (blood flow QB, dialysate flow QD)
  • Evaluate intradialytic problems, intervene as prescribed, monitor patient response to therapy
  • Obtain vital signs prior to reinfusion, discontinue treatment per procedures, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre/post evaluations, weights, vital signs
  • Calculate and enter patient-specific dialysis programming (UF goal, time, modeling), ensure safety alarms enabled
  • Record vital signs and treatment parameters on Hemodialysis Treatment Sheet
  • Document patient treatment information, complete Treatment Sheet and progress notes
  • Report significant information/changes in patient condition to RN or supervisor
  • Conduct machine safety checks, document interventions for unusual patient status
  • Monitor vascular access during treatment (arterial/venous pressures), intervene/report unusual findings
  • Obtain prescribed lab testing, prepare specimens, clean blood spills per policy
  • Prepare/organize supplies and equipment to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff on programs
  • Maintain safe/clean working environment
  • Support Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements
