Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to setup and operate hemodialysis machines, including programming for Ultrafiltration (UF) goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits 160)
  • Skills in performing vital signs, weights, vascular access evaluation, cannulation, initiating/discontinuing dialysis, monitoring intradialytic problems, hemostasis, dressings, and laboratory specimen preparation
  • Ability to maintain accurate documentation on Hemodialysis Treatment Sheet and progress notes
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvement, and patient satisfaction initiatives
  • Maintenance of safe and clean working environment, machine safety checks, blood spill cleanup

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate, administer heparin
  • Obtain pre- and post-treatment vital signs, weights, and vascular access evaluations
  • Initiate dialysis per orders (QB, QD), monitor patient response, evaluate intradialytic problems, intervene as prescribed
  • Discontinue treatment, obtain reinfusion vital signs, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre/post evaluations
  • Calculate and enter patient-specific dialysis programming (UF goal, time, modeling), ensure safety alarms
  • Document vital signs, treatment parameters, interventions on Hemodialysis Treatment Sheet/progress notes
  • Report significant changes or unusual findings to RN/supervisor
  • Conduct machine safety checks, monitor vascular access (arterial/venous pressures), intervene as needed
  • Obtain prescribed lab testing, prepare specimens, clean blood spills
  • Prepare/organize supplies/equipment to prevent wastage
  • Monitor self-care patients under RN supervision
  • Assist in training direct patient care staff on applicable programs
