Requirements

  • Must have dialysis experience
  • Registered Nurse (RN) license
  • Meets RN CAP 2 criteria (entry level or advancement from RN CAP 1)
  • Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Complete CAP requirements to maintain or advance

Responsibilities

  • Perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust treatment plan as indicated, and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery
  • Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource, leader, coach, mentor, and role model for new and incumbent employees
  • Ensure correct laboratory collection, processing, shipping, and reschedule missed/insufficient collections
  • Identify expected outcomes, document, and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Participate in education and quality improvement projects at facility and area level
  • May serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
  • Perform all functions under direction of Supervisor and with guidance from Clinical Educator, Preceptor, or in collaboration with other RNs, in accordance with policy, procedures, nursing standards, and regulations
