Entry level designation into the Clinical Advancement Program (CAP) - required to complete CAP requirements to advance
Ability to perform direct patient care involving heavy lifting, moving of patients, assisting with ambulation, frequent prolonged standing, bending over, moving machines/equipment up to 200 lbs. (with assistance), and lifting chemical/water solutions up to 30 lbs. as high as 5 feet
Capability to work in a health care facility environment with air temperature control, moderate noise levels, potential exposure to infectious/contagious diseases/materials
Performs duties under direction of Supervisor and with guidance from Clinical Educator/Preceptor or in collaboration with other RNs
Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations

Responsibilities

Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership functions, and quality improvement activities as part of the End Stage Kidney Disease (ESKD) health care team
Ongoing systematic collection and analysis of patient data pre-during-post hemodialysis treatment, documentation in medical record, adjustments/modifications to treatment plan, and notification to Team Leader/Charge Nurse/Supervisor/Physician as needed
Assessment, collaboration, and documentation of patient/family basic learning needs; provision of initial and ongoing education
Direction and provision (in collaboration with staff) of all aspects of daily safe and effective hemodialysis therapy delivery
Administration of medications as prescribed or per approved algorithm(s), with documentation of justification and effectiveness
Initiation or assistance with emergency response measures
Serving as a resource for health care team, participation in staff training and orientation of new staff
Ensuring correct laboratory collection, processing, and shipping; rescheduling missed/insufficient collections
Identification of expected outcomes, documentation/updating of nursing assessment and plan of care via collaboration with Interdisciplinary Team
Ensuring patient awareness of transplant and treatment modality options
Performs all other duties as assigned by Supervisor
