Requirements

  • Functions as part of the hemodialysis health care team as a Staff Registered Nurse
  • Adheres to FMS policies, procedures, training, FMCNA Compliance Program, and patient care/administrative policies
  • Supports FMCNA’s mission, vision, values, customer service philosophy, Quality Enhancement Program (QEP), CQI activities, and process improvement for patient satisfaction and QEP goals
  • Holds active Registered Nurse (RN) licensure (implied by job title and duties)

Responsibilities

  • Directs Patient Care Technicians in safe and effective delivery of chronic hemodialysis therapy per facility policies, corporate, state, and federal regulations
  • Delegates tasks to direct patient care staff (e.g., LVN/LPNs, Patient Care Technicians, Dialysis Assistants)
  • Assesses daily patient care needs and develops appropriate patient care assignments
  • Routinely monitors patient care staff for appropriate techniques and adherence to policies/procedures
  • Participates in staff training, orientation of new staff, and required staff meetings
  • Ensures educational needs of patients and families are met regarding End Stage Renal Disease (ESRD), renal disease, vascular access, dialysis therapy, related health conditions, diet/fluid/medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, laboratory values, and dialysis compliance
  • Provides safe and effective delivery of care to ESRD patients, including accurate implementation of treatment prescriptions (e.g., Sodium modeling, Ultrafiltration modeling)
  • Assesses patients’ response to hemodialysis, makes adjustments/modifications per physician, and communicates issues to Team Leader or physician
  • Identifies and communicates patient-related issues to Team Leader or physician
  • Initiates Initial and Annual Nursing Assessments, ongoing evaluation, and documentation of patient care needs per FMC policies
  • Actively participates in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatments
  • Takes intervention for changes in patient adequacy status and troubleshoots access flow issues (e.g., OLC/AMP yellow lights)
  • Provides, supervises, and monitors hemodialysis access care per procedures
  • Implements, administers, monitors, and documents responses to prescribed interdialytic transfusions, notifying physician and blood supplier of adverse reactions
  • Ensures accurate and complete documentation by Patient Care Technicians on Hemodialysis Treatment Sheets
  • Reviews, transcribes, and enters physician lab orders into Medical Information System
  • Ensures appropriate preparation of lab requisitions for Spectra or alternate labs
