Requirements

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
  • Ability to function under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including setup and operation of hemodialysis machines
  • Skills in vascular access evaluation, cannulation, and heparin administration as allowed by state law
  • Ability to perform vital signs monitoring, blood pressure measurements, and patient evaluations
  • Proficiency in dialysis treatment initiation, monitoring, discontinuation, and documentation
  • Capability to calculate and program dialysis machine parameters (e.g., UF goal, treatment time, UF modeling)
  • Understanding of safety alarms (e.g., Narrow Venous Limits at 160) and machine safety checks
  • Ability to assist in patient education on adequacy measures (e.g., OLC, AMP, UKM) and disease processes

Responsibilities

  • Assist in patient education on adequacy measures, disease process, and vascular access
  • Welcome patients, inquire about wellbeing, and evaluate vascular access patency
  • Perform pre- and post-treatment vital signs, weights, and vascular access evaluations
  • Cannulate vascular access and administer heparin as delegated
  • Initiate dialysis treatments per prescribed orders (e.g., QB, QD flows)
  • Monitor patients during treatment, evaluate intradialytic problems, and intervene as prescribed
  • Discontinue treatments, obtain hemostasis, apply dressings, and evaluate patients prior to discharge
  • Calculate and enter patient-specific dialysis programming (UF goal, time, modeling)
  • Ensure safety alarms are enabled and perform machine safety checks
  • Document all treatment parameters, vital signs, evaluations, and interventions accurately and timely on Hemodialysis Treatment Sheet and progress notes
  • Report significant patient changes or unusual findings to RN or supervisor
  • Obtain and prepare laboratory specimens
  • Clean blood spills and maintain a safe, clean environment
  • Prepare and utilize supplies/equipment efficiently to prevent wastage
  • Monitor self-care patients under RN supervision
  • Assist in training direct patient care staff
  • Participate in Quality Enhancement Program (QEP), CQI activities, and process improvements for patient satisfaction and goals
