Requirements

Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license
Ability to function as part of the hemodialysis health care team under direct supervision of a registered nurse
Flexibility to float and provide coverage across multiple clinics in the Madison area and vicinity
Adherence to FMCNA Compliance Program, regulatory requirements, division/company policies, mission, vision, core values, and customer service philosophy
Ability to report and document all new or unusual incidents, information, complaints, or problems to the supervisor
Commitment to promoting and maintaining a safe and clean work environment

Responsibilities

Drive FMCNA culture through values and customer service standards, providing outstanding customer service to all external and internal customers
Develop and maintain effective relationships through effective and timely communication
Take initiative to respond, resolve, and follow up on customer service issues in a timely manner
Provide direct patient care for assigned patients
Perform all technical aspects of dialysis procedures for assigned patients as prescribed
Administer and monitor treatments in accordance with the current treatment plan or as directed by the supervisor
Monitor and document dialysis treatment parameters on dialysis flow sheets
Document other information related to patient care in the individual patient record
Report any significant information or change in patient condition
