Requirements

Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between May 2024 and September 2025
Currently residing in or willing to relocate to program location for at least the next two years
Have not yet worked as an NP
Minimum of one year experience, within the last five years, working as an adult RN
Minimum GPA of 3.0 in NP program
Currently holds or will hold by 3/16/2026 an active, non-probationary state NP license
Spanish required (per job title)

Responsibilities

Commit to working at Oak Street for at least a year following program completion
Build your own patient panel during the 12-month fellowship
Learn from a multitude of preceptors
Engage in didactic content to build on graduate training
Participate in the fellowship program in formal collaboration with the University of Michigan School of Nursing, focused on adult primary care in a value-based care model for older adults
