Requirements

  • Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between May 2024 and September 2025
  • Currently residing in or willing to relocate to program location for at least the next two years
  • Have not yet worked as an NP
  • Minimum of one year experience, within the last five years, working as an adult RN
  • Minimum GPA of 3.0 in NP program
  • Currently holds or will hold by 3/16/2026 an active, non-probationary state NP license
  • Spanish required (per job title)
  • A genuine passion for adult primary care

Responsibilities

  • Commit to working at Oak Street for at least a year following program completion
  • Build your own patient panel during the 12-month fellowship
  • Learn from a multitude of preceptors
  • Engage in didactic content to build on graduate training
  • Participate in the fellowship program in formal collaboration with the University of Michigan School of Nursing, focused on adult primary care in a value-based care model for older adults
