Requirements

Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between May 2024 and September 2025
Currently residing in or willing to relocate to program location (Rio Bravo - 1625 Rio Bravo Blvd SW Suite 36, Albuquerque, NM 87105) for at least the next two years
Have not yet worked as an NP
Minimum of one year experience, within the last five years, working as an adult RN
Minimum GPA of 3.0 in NP program
Currently holds or will hold by 3/16/2026 an active, non-probationary state NP license

Responsibilities

Commit to working at Oak Street Health for at least one year following program completion (with a second year of employment required post-fellowship)
Build your own patient panel during the 12-month fellowship
Learn from a multitude of preceptors
Engage in didactic content to build on graduate training
Participate in the value-based care model focused on quality of care for older adults
