Responsibilities
- Perform in-home visits to assess a patient’s overall health and well-being
- Conduct a physical evaluation of the health plan member in their home
- Review medication and medical history
- Identify potential barriers to care
- Use pre-populated, patient-specific forms for each evaluation
- Do not prescribe medicine, order lab tests, or alter existing treatment regimens
